Admission Procedure

The following information will need to be provided the day of or prior to admission:

1. Completed Medical forms such as:

  • History and Physical exam (no more than 5 days old)
  • Physician’s orders
  • Results of a chest x-ray (no more than 90 days old)
  • OBRA screening form (MIMI)-Mental health screening form
  • Physical, Occupational, and Speech therapy notes, if applicable
  • Transfer sheet from hospital or any other health care center

2. All health insurance cards (Medicare, Medicaid, Blue Cross etc.), and Social Security card

3. Proof of: Power of Attorney, Guardianship, Conservator, Health Care Surrogate or Durable Power of Attorney

4. Living Will/Advance Directives

5. Washable clothing and any comforting or familiar items from home. Sanctuary at Saint Mary's’s Laundry Department will professionally label all clothing upon admission.

A resident cannot be admitted from home without all necessary medical forms. To assure a current History and Physical, an appointment with the patient’s attending physician may be required.

A resident will be assigned to a physician prior to admission. If a resident’s attending/family physician wishes to be responsible for following the resident at Sanctuary at Saint Mary's, arrangements should be made prior to admission. State law requires that the resident’s attending physician see the resident within 48 hours of admission, every 30 days for the first 90 days and a minimum of every 30 to 60 days thereafter.
