Summary of Benefits for Hourly Employees

Vacation Trinity Continuing Care Services

Full and part-time employees (16+ hours/week)

Effective: After 1 year of continuous service

Vacation time is awarded based on hours paid to employee in previous calendar year and length of service as follows:

1 year to < 3 years 5 days
3 years to < 6 years 10 days
6 years to < 11 years 15 days
11+ years 20 days

• Employee may bank unused time to a maximum of 1.5 times annual accrual.
Personal Days Trinity Continuing Care Services

Full-time employees

Effective: After 90 days of service

One (1) pre scheduled Personal Day per calendar year.
Sick Days Trinity Continuing Care Services

Full and part-time employees (20+ hours/week)

Effective: After 6 months of service

Full-time employees accrue a maximum of 4 days per year. Part-time employees accrue days based on prorated number of hours paid. Maximum accrual 64 hours.
Holiday Trinity Continuing Care Services

Full-time, employees

Effective: Date of hire

Eight (8) paid holidays per year: New Year’s Day, Easter, Memorial Day, 4th of July, Labor Day, Thanksgiving, Christmas, and Employee’s birthday.
Premium Conversion Account Employee Participants in the medical & dental plans. Pre-tax salary reduction for required medical and dental premiums.
Medical Benefit Plans Trinity Continuing Care pays 75%* of full premium for all levels of coverage. Employee pays 25% by payroll deduction.

Full-time employees

Effective: 1st of the month following date of hire.

Blue Care Network (HMO). Premiums are pre-tax salary reduction.
Dental Employee pays 100% of single, dependent or family coverage by payroll deduction.

Full-time employees

Effective: 1st of the month following 90 days of service

Delta Dental available. Premiums are pre-tax salary reduction.

Spectera Vision Plan provides coverage for eligible vision expenses such as annual eye examination, lenses, frames, and contact lenses subject to co-pays and plan limits.
Group Term Life Trinity Continuing Care Services

Full and part-time employees

Effective: 1st of the month following 90 days of hire

Basic life insurance in the following coverages:

• Employees working 30+ hours/week: 2x annual salary
• Employees working less than 30 hours but more than 16 hours/week: 1x annual salary.

Accidental Death & Dismemberment: 1x annual salary

Group Legal Employee after-tax salary reduction Effective: 1st of the month following 90 days of hire Group Legal Plan, sponsored by Hyatt Legal Plans, provides coverage for legal advice and assistance for most personal legal matters (employment and business-related matters are excluded from coverage). The Plan does not impose waiting periods or co-payment requirements.
Short-term Disability Trinity Continuing Care Services

Full and part-time employees (20+ hours/week)

Effective: After 90 days of service

Payment will be made for up to 6 weeks for sickness or accident as follows:

Trinity Continuing Care Services

Full and part-time employees

Effective: Date of hire

Monthly retirement benefits from Trinity Retirement Plan based upon final average earnings, years of service and age when retirement benefits (must be vested at termination). Vesting occurs after 5 calendar years of service, with a minimum of 1,000 hours worked.
Trinity Retirement Savings Plan Employee salary deduction

Full and part-time employees

Effective: Date of hire

Employee may participate in a voluntary tax deferred payroll deduction retirement savings program using Tax Deferred Annuities 403(b). A number of different investment options are available.
Trinity “Match” Plan Trinity Continuing Care Services

Full and part-time employees

Effective: Date of hire

If employee participates in the Trinity Retirement Savings Plan, Trinity will “match” the employee’s contribution, 100% for the first $500, then 50% on additional up to a maximum “match” of 2% of the employee’s taxable compensation. “Match” is paid on quarterly basis. Vesting occurs after 3 calendar years of service.
Tuition Reimbursement Trinity Continuing Care Services

Full and part-time employees

Effective: After 1 year for non-clinical studies and 6 months for clinical studies

Trinity Continuing Care Services will reimburse 100% of tuition, books, and course related fees up to a maximum of $2,500 per calendar year for full-time employees and $1,250 per calendar year for part-time employees who work more than or equal to 25 hours per pay period.

Employees completing clinical studies in fulfillment of the requirements of LPN, BSN or RN program will receive up to a maximum of $1,250 per calendar year.

Leaves of Absence Non-paid. Disability leaves receive short-term disability.

Full and part-time employees

Effective: After 90 days of service

Trinity Continuing Care Services shall consider and/or approve leaves of absence requests for the following reasons: Family Leave, Medical Leave, Personal Leave, Education Leave, Military Duty Leave, and Union Duty Leave.
Bereavement Trinity Continuing Care Services

Full and part-time employees

Effective: After 90 days of service

For immediate family, up to three (3) consecutive paid days off. Two (2) consecutive days off for mother-in-law, father-in-law, or grandparents.

Jury Duty Trinity Continuing Care Services

Full and part-time employees

Effective: Date of hire

Employees will be paid for regular hours of work missed minus the difference in the daily jury duty fee.
Employee Assistance Program Trinity Continuing Care Services

Full and part-time employees

Effective: Date of hire

Employee and family members may seek free, confidential counseling to assist with personal challenges.
